A closed change ticket is evidence that a task was completed, not proof that the vulnerable condition disappeared. Failed installations, pending reboots, duplicate assets, and unexpected dependencies can leave a finding unresolved.
Define success before patching. Record the affected asset, installed version, vulnerable condition, remediation owner, and how the change will be checked.
After the change window, inspect version and configuration state and run an appropriate verification scan. Investigate persistent findings rather than automatically marking them as false positives.
Keep remediation and exception evidence together. When a patch cannot be applied, document approved compensating controls, a responsible owner, and an expiration date for the exception.
